Medical Claims

Seib has agreement with certain medical providers that allow direct billing facilities. You may elect to use any of them unless there is a specific reference to an exclusion in the benefits table. In Such cases, you simply present your card, pay any applicable deductible and receive your treatment.

Present your Seib Access Card and ID Card
Pay the upfront co-payment or deductible
Follow Provider Instructions To Receive Treatment
All documents should be submitted in English or Arabic
Claims must be submitted within 60 days from first day of treatment if treatment is in Qatar and 90 days for that outside Qatar
Requested pending documents to be submitted within 90 days if treatment is in Qatar and 120 Days for that outside Qatar
Submission routs:

1) To your broker or agent
2) Directly to Seib

Seib claim form
Discharge summary (for in-patient treatment)
Medical report (for out-patient treatment)
Itemized original invoices
Lab results if applicable
Radiology results if applicable
Medication prescription (if applicable)
Optical/Dental prescription (for optical/dental claims)

Your claim status and payment will be communicated to you through our Seib Customer Care Professionals.
